Social Prescribing · Hampton & Newport News
Connections that hold between appointments.
For case managers, reentry programs, clinics, and recovery residences. Refer someone. We connect them to nature-based groups and community support, and report back.
What it is
Support that fits a person's life.
Much of what keeps a person well happens outside an office. Social prescribing connects someone to community supports that fit their life, then reports back to the person who referred them.
It is the model England's National Health Service uses in primary care, adapted here for Hampton and Newport News.
Nature comes first. The NHS calls this green social prescribing: walking groups, community gardens, and other nature-based activities. Ours begins with a guided walk.
Non-clinical. We work alongside treatment and case management, never in place of it.
What we connect people to
- Guided nature walks and outdoor groups, first
- Peer support groups
- Volunteering outdoors, like our annual Harbor Our Shore cleanup
- Education and job training resources
- Benefits they qualify for
- Community groups of their choosing
How it works
Four steps, start to finish.
You refer
A short email with the basics and the person's consent. The referral button starts it for you.
We meet
A "what matters to you" conversation, in person or by phone.
We connect
Starting with a guided nature walk when it fits, then the other supports, with a warm handoff instead of a phone number.
We follow up
Every connection is documented and, with consent, shared with you.
Who refers
Built for the people who send people.
Referral partners receive a short monthly summary of referrals and connections, ready to drop into your own reporting.
